Doctor Alvarezs point is well taken. Increasing evidence over the past few years has taught us a great deal about improving the viability of the latissimus dorsi muscle (LDM).
Much of the recent research was instigated only after pioneering surgical work had made cardiomyoplasty a realistic option for heart failure. There may, indeed, now be better ways of preparing the LDM, but we should remember that the early results of cardiomyoplasty had suggested that there was little problem with the muscle flap.
In answer to the question, yes, we believe these findings deserve clinical application.
