Undetected cold agglutinins were encountered in a patient undergoing
coronary artery bypass grafting using antegrade cold blood cardioplegia.
Macroemboli were infused into the coronary arterial tree. These were
flushed out by the use of retrograde cold crystalloid cardioplegia.
Activated cold agglutinins may be hazardous in cold blood cardioplegia.
