Acute myocardial ischemia and infarction due to retrograde coronary artery
dissection are uncommon complications after ascending aortic dissection. A
case of acute aortic dissection with massive right ventricular failure
successfully treated by replacement of the ascending aorta and
postoperative right ventricular mechanical support is presented. Although
mechanical assistance may be hazardous after ascending aortic replacement,
because of the need of anticoagulation, it represents a valid therapeutic
option when severe right ventricular failure due to acute myocardial
infarction ensues.
