This report describes a recipient of single-lung transplantation surviving
extraordinary complications: (1) early graft failure mandating
retransplantation; (2) left atrial thrombus formation, which resolved by
recombinant tissue plasminogen activator lysis; (3) and development of a
"locked-in-syndrome." Possible underlying mechanisms are discussed.
